Asuhan keperawatan dilakukan dengan wawancara kepada pasien, tindakan keperawatan dilakukan langsung kepada pasien dengan di dampingi oleh perawat. Pasien dibawa ke rumah sakit karena pasien mendengar suara yang menyuruhnya untuk menyakiti dirinya sendiri saat sendirian dan sore hari, tidak menentu berapa kali dalam sehari, gelisah dan takut saat mendengar suara tersebut, diagnosa yang muncul pada pasien yaitu harga diri rendah, isolasi sosial, defisit perawatan diri, gangguan persepsi sensori : halusinasi pendengaran dan resiko perilaku kekerasan. Perencanaan sudah sesuai dengan standar rumah sakit. Evaluasi yang telah dicapai yaitu pasien dapat menghardik, minum obat, bercakap cakap dengan orang lain dan melakukan aktivitas terjadwal. Kata kunci : Asuhan Keperawatan dan Halusinasi Hallucinations are one of the symptoms of sensory perception disorders experienced by people with mental disorders. The impact of impaired sensory perception of hallucinations is the loss of self-control which can cause a person to panic so that his behavior is controlled by his hallucinations and besides the effects of hallucinations can also hurt themselves or others. Nursing care is carried out by interviewing patients, nursing actions are carried out directly to patients accompanied by nurses. The patient was brought to the hospital because the patient heard voices telling him to hurt himself when alone and in the evening, was erratic at how many times a day, was nervous and scared when he heard the sound, the diagnoses that emerged in the patient were low self-esteem, social isolation, deficits self-care, impaired sensory perception: auditory hallucinations and risk of violent behavior. Planning is in accordance with hospital standards.
